You are responsible for any costs that insurance will not cover, as well as any copays for services, and other unpaid balances.

An active credit card must be kept on file at all times, except for Medicaid clients.

I ask for a minimum of 48 hours notice to be given for any appointment changes or cancellations, outside of illness or emergency.

If you do not give adequate notice for visit cancellations, you are still responsible for the cost for services (non-Medicaid clients). Unexcused visit no-shows are cause for termination of therapy services.
