Currently accepting UHC, Cigna, BCBS, Molina/UHC Medicaid, Aetna, Midlands Choice, HSA & self-pay.
Affordable sliding scale fee services may also be available depending on availability-- ask for more info! :)
You are responsible for any costs that insurance will not cover, as well as any copays for services, and other unpaid balances.
An active credit card must be kept on file at all times, except for Medicaid clients.
Minimum of 48 hours notice to be given for any appointment changes or cancellations, outside of illness or emergency.
If you do not give adequate notice for visit cancellations, you are still responsible for the cost for services (non-Medicaid clients). No call/no-shows are cause for termination of therapy services.
